Home Medication Visits
Ideal for individuals who:
– Struggle with cognitive disorders that make it difficult to remember to take medications
– Have challenges managing or administering certain types of medications, such as injectables (like insulin), inhalers, or eye drops
– Experience difficulty maintaining stable glucose or blood pressure levels despite multiple therapies, often due to inconsistent or inaccurate monitoring
This service is covered by BC Fair PharmaCare (deductible may apply, see fee/FAQ page for more information).
Daily/Regular Home Visiting
Our team of professional nurses will visit you—typically once a day—to provide medication support in the comfort of your own home.

Our team—consisting of pharmacists, pharmacy technicians, nurse supervisors, and pharmacy assistants—is dedicated to supporting all your medication care needs, from identifying potential drug interactions to managing titrations and complex drug therapies.
What to expect during our visits
Step 1.
Oral medications witnessing
We will organized your medications into a single daily strip, sorted by the times of the day they need to be taken. One strip will be delivered at each daily visit.
- One day supply is provided at a time to prevent overdose.
- Pharmacist will arrange medications to the time best suited.
- Each strip contains the specific date and names of all medications to reduce administration error.
**The dispensing of oral medications is a requirement if you would like to receive government coverage for our program.
Step 2.
Other medications types administration
As the visits are done by licensed nurses, they can do a variety of administration to help those with fear of needles or just simply have a hard time with tricky medical device.

Step 3.
Health signs measurements
Following the prescriber's orders, our nurse will measure and document various health signs that are important in optimizing your drug therapy, such as
- Blood Pressure
- Blood Sugar (important for insulin-users)
- Weight (certain drugs are dosed based on the weight of the day)
- Blood Oxygen
Step 4.
Regular monitoring report to your prescribers
Collected health data will be timestamped and compiled into a monitoring report, which will be regularly shared with your prescribers.
- This allows your prescriber to make timely adjustment to your medications as needed. These changes will be promptly reflected in your blisterpack.
- Ongoing monitoring can help identify potential health issues early, reducing risk of complications or hospitalization.
Step 5.
Ongoing interprofessional care for better outcome
Through regular in-home visits and close collaboration with you, your family, and your prescriber, we aim to support medication adherence and timely adjustments in order to:
- Reduce risk of hospitalization
- Support your independence at home
- Delay the need for long term care facility
